If you wish to request a Career Equity Review (CER), please complete this form and submit it to your Department Chair or to your Dean no later than May 15 of the year prior to the review. Also send a copy to the Academic Personnel Office.

Please include a statement of two pages or less that indicates why you believe that the current rank and step are inappropriate and what you believe is appropriate.

I am aware that past personnel actions may be viewed during the review process.
